IMPORTANT NOTICE: Unencrypted email is not secure. It may be intercepted, read, redistributed, or stolen at many places between the source and destination. Sending unencrypted patient names, SSNs and other identifiable information via email is a violation of HIPAA and it exposes this information to compromise and potential theft of the patient's identity. Please describe the issue in email and include your phone number along with any relevant data other than patient identifiers. The SRTR will then follow-up via telephone if we require patient identifiers. Data requests containing other patient identifiable information will not be processed.
